The Patient's current use of drugs should be entered into the Complete Healthcare Record (CHR) periodically or as needed. Entries should include responses and reactions to the drugs.

A Patient's Practitioner(s) should be charged with the responsibility of reviewing and responding to the information provided in the CHR.
