Information that potentially impacts a diagnosis and subsequent treatment, plus the Patient's responses, should be entered into the Complete Healthcare Record (CHR).

The diagnosis and subsequent treatment provided by Dentists, and the responses, can have wide-ranging impacts on other diagnosis and treatments and hence are important entries in a CHR.

A Patient's Practitioner(s) should be charged with the responsibility of reviewing and responding to the information provided in the CHR.
