Discussion
Information that potentially impacts a diagnosis and subsequent treatment,
plus the Patient's responses, should be entered into the
Complete Healthcare Record (CHR).
The relationship between a Patient's
Practitioner(s) and the Patient's
Healthcare Caregiver(s) is often overlooked and avoided by
the Practitioner(s). Although such relationships impose
additional burdens the Healthcare Caregiver is a
source of Healthcare-related information that may at times
be very important and perhaps crucial.
The Healthcare Caregiver should be provided the opportunity
to generate and update EHRs related to the Patient's
Healthcare they have contact with or knowledge of. As a potential
source of Healthcare-related information, irregardless of
potential redundancies, the
Complete Healthcare Record should contain their information.
A Patient's Practitioner(s) should be charged
with the responsibility of reviewing and responding to the information
provided in the CHR.
