Discussion
A Patient should have a
Complete Healthcare Record (CHR)
that starts prior to birth and continues
until death. The CHR should include all pertinent
information from all events that shape the Patients
Healthcare as well as information provided by the
Patient, the extended and
immediate family, and all
Patient Support Groups.
Information regarding the Patient's Healthcare and
related impacts throughout life should be contained within the CHR.
After death information contained within the CHR
may prove invaluable for subsequent Healthcare, Public Health and
family-related research.
A Patient's Practitioner(s) should be charged
with the responsibility of reviewing and responding to the information
provided in the CHR.
