DiscussionPatient EHRs contain information that may be crucial to a diagnosis and subsequent course of treatment. The attending Practitioner and/or the Healthcare Facility may or may not have access to the CHR or a pertinent subset thereof. It is the Patient's responsibility to ensure that the appropriate Practitioners and Facilities has a sufficient portion (derivation) of the CHR upon which a proper diagnosis and course of treatment can be based. The Practitioners and the Facilities have a responsibility to ensure that Patient's privacy and security is protected at all times. This includes periods in which the Patient is not seeking Healthcare services from the Practitioners and/or the Facilities. This extends to proper handling and disposition of derivations of the CHR made available by the Patient. It is the Patient's responsibility to merge derivations into the CHR. The Practitioner's and the Facilities need to integrate Healthcare-related information into the CHR justifies a requirement that the appropriate Practitioners and Facilities be granted special access to a derivation of the CHR created by the Patient for them. Practitioners are responsible for reviewing and responding to information contained within the CHR. The Healthcare Facility should also review and respond to appropriate and related information within the CHR. |
